Patients who have the following long-term conditions will be given appointments to see the Practice Nurse (and/or a GP if appropriate) for a review at least annually. It is important that you attend your appointment, even if you feel well. The reviews may include blood tests.

Heart Disease

Patients with heart disease should be reviewed at the practice at least annually. The Practice Nurse will arrange all relevant blood tests and have these available to review with you at your appointment.

Stroke & TIA

Patients who have previously suffered a Stoke or Transient Ischaemic Attack (TIA) will be asked to attend for regular review so that we can optimise the treatment of your condition.

Diabetes

The Practice offers diabetic reviews at a special appointment twice each year, where we will discuss diabetes management with the patient and go through any concerns they may have.

Hypertension

Patients will be reviewed by the Practice Nurse, with a GP available for consultations and examination, if necessary. Patients are seen for blood pressure monitoring and medication checks.

COPD

COPD reviews allow our Nurse to check your height, weight, blood pressure and medication. A spirometry test is also carried out to determine if there is any deterioration in the patient’s lung function.

Asthma

Asthma clinics are run by our Practice Nurse, where we will assess your treatment and inhaler therapy to ensure your are on the correct medication to help keep your asthma in control.

Epilepsy

Epilepsy is a chronic condition that needs frequent review and our patients are managed and closely monitored by the Practice Nurse, whether for urgent ailments or for regular health checks.

Hypothyroid

Yearly checks are carried out by our Pharmacists, which include the necessary measurements and blood tests, to make sure that patients are treated optimally for thyroid problems.

Mental Health

The Practice invites all Mental Health sufferers into for a general health check where they have a thorough consultation to discuss concerns and treatment with the Patient and/or their carers.

Cancer

Patients who have suffered with cancer will be monitored and followed-up by our Primary Care Team who are here to help advise and treat on your general health as well as cancer-related problems.

Kidney Disease

Patients with chronic kidney disease or acute kidney injury are monitored with regular blood tests and general health reviews in the practice to help prevent any further deterioration in their condition.

Depression

One in every five people in the UK suffer from depression. Our team are able to help by providing friendly advice and your GP can also refer you for the best choice of therapy to help with your condition.

Atrial Fibrillation

If you suffer from Atrial Fibrillation, our Pharmacists and Nurses will regularly monitor you with necessary tests, such as ECG, anti-coagulation, and blood tests, and will help you manage your symptoms.

Obesity

Your GP can advise you about losing weight safely by eating a healthy, balanced diet and regular physical activity, and our Nurses can help you monitor any changes you make to your lifestyle.

Learning Difficulties

People with learning disabilities often have poorer physical and mental health than other people, and are therefore reviewed at least annually in the Practice by our Doctors.

INR/DOAC Monitoring

For patients who are taking Warfarin or anticoagulant medication, we carefully monitor their blood test results at regular intervals and adjust your medication to make sure it’s having the right effect.
